APPLICATIONS

The Institutional Review Board (IRB) is a standing committee at Kentucky State University established in compliance with federal policy for the protection of human rights in research.

Institutional Review Board (IRB) Exempt Application (Form)

Description: Request to conduct research in established or commonly accepted educational settings, involving only normal educational practices (regular or special education instructional strategies, effectiveness of instructional techniques, curricula, classroom management methods). Do not use if research is federally funded or if compensation is involved.


Institutional Review Board (IRB) Expedited Application (Form)

Description: Unlike exempt review, expedited review falls under the full protection of regulations and are reviewed administratively by the IRB Exempt/Expedited Team.


Institutional Review Board (IRB) Full Review Application (Word Doc)

Description: Studies that are not eligible for expedited review (do not meet the definition of minimal risk and/or do not fit into an expedited category) must be reviewed by the convened IRB.  


The IRB protects the rights and interests of human research subjects for research conducted under the auspices of the University. All research projects involving human subjects that are conducted at the University and/or by University personnel, regardless of whether or not it is externally funded, must be reviewed and approved by the IRB.

Proposals involving human subjects should be submitted to the Office of Grants and Sponsored Programs for assignment to the IRB, along with completed forms corresponding to the type of research being proposed. Please note that the IRB cannot review research that has already been performed or is being performed.  Data collected prior to IRB approval cannot be used. All applications should be submitted electronically via the KSU IRB submission portal button below. Hard copies of applications are not accepted. All files should be in Microsoft Word or PDF format.

Please contact the Office to discuss your project, and we will provide the forms that you need. The Office can also provide a model consent form that researchers can adapt and use as needed to document informed consent of human subjects. Principal Investigators should allow at least 30 days for completion of IRB review.

Following IRB review, the Principal Investigator will receive a response in writing regarding approval, disapproval, or suggestions for modifying the proposed protocol and resubmission.


INSTITUTIONAL ANIMAL CARE AND USE COMMITTEE (IACUC)

The Institutional Animal Care and Use Committee is responsible for ensuring that research institutions comply with federal laws and guidelines when using animals. They oversee all animal-related activities, including research, teaching, and experimentation.

IACUC provides oversight of and reviews the care and use of vertebrate animals in teaching, testing, and research at the University, in accordance with the Animal Welfare Act. More information about the Animal Welfare Act and its guidance regarding IACUC can be found here:
