Submit a Service Request

If this is an urgent request, please call Facilties at (502) 597-6805.

Please note that two (2) weeks’ notice is required for all set-up requests (special event, moving office, office assignment, etc.)

Request to perform the service should be coordinated within the administrative unit of your organization. This will help prevent duplication of the service request.

Be prepared to provide the following information:

  • Your email address where we will send confirmation
  • Requester name and telephone number (and contact person if different from caller).
  • Building name, room number or specific location where the service is needed.
  • Description of service needed (please be specific when describing the problem).
