Upward Bound Application
In order for the application to be complete, we will need the following documents;:
- Completed application (Please be sure to get two (2) different teacher recommendations).
- Proof of free and/or reduced lunch (Please include this document if your child qualifies.)
- Standardized test scores
- Recent report card
- Discipline report
