FERPA - Student Authorization Form

The Family Educational Rights and Privacy Act (FERPA) grant students certain rights concerning his/her education records.

Notwithstanding these rights, the student may release his/her educational records to parent(s) or guardian(s) for the purpose of keeping them informed about the student's education at Kentucky State University.

Education records include, but are not limited to, information about student academic standing, disciplinary issues and financial obligations to Kentucky State University.

This consent will remain in effect until the student graduates or withdraws from the Kentucky State University.

The student may submit a subsequent notification in writing directing Kentucky State University to no longer release information to any or all of the authorized individuals.
