Data Breach Update: Tax information

The University has received information that the individual or individuals who fraudulently obtained Kentucky State University (“KSU”) W-2 information as a result of the recent data breach have contacted the Internal Revenue Service (“IRS”) and requested tax information about KSU employees.

If you were affected by the data breach, you need to contact the IRS as soon as possible to notify the agency that you are a victim of identity theft. You may do this by completing the attached IRS Form 14039, Identity Theft Affidavit. The attached fillable form also is available at https://www.irs.gov/pub/irs-pdf/f14039.pdf

Complete the form, print and mail or fax it to the IRS according to the instructions on the form. If you use a tax preparer to complete your taxes, please notify them that you are a victim of identity theft as well.
