Shot for Bred vaccine incentive program
Kentucky State University officials encourage all members of its community to get vaccinated against COVID-19. Students, faculty and staff who provide a record of their COVID-19 vaccination information can enter to win prizes with a series of drawings starting Jan. 17, 2022.
How to enter
Entry into the drawing must be completed by registering for the incentive drawing. If you were vaccinated at a Kentucky State University affiliated site, you do not need to self-report unless asked to do so by Student Health Services. Entry requires agreement to the official rules of the drawing and agreement to allow Kentucky State to verify your student status, vaccination information, and eligibility to receive the prize prior to prize dispersal.
Students who are enrolled in one or more courses at Kentucky State who have received at least their first dose of a Moderna, Pfizer or other World Health Organization approved 2-dose COVID-19 vaccine, or the one-dose Johnson & Johnson or other World Health Organization approved single-dose vaccine, may enter to win these drawings.
Campus community members who do not have access to the internet may contact Student Health Services at (502) 597-6271 for assistance entering the drawing.
Eligibility and verification
All Kentucky State University students are eligible for the drawing if they:
- Are enrolled in one or more course(s) at Kentucky State University at the close of business on the day prior to the drawing; and
- Have received at least their first dose of a Moderna, Pfizer or other World Health Organization approved 2-dose COVID-19 vaccine, or the one-dose Johnson & Johnson or other World Health Organization approved single-dose vaccine; and
- If you were vaccinated at a Kentucky State University affiliated site, you do not need to self-report unless asked to do so by Student Health Services.
All Kentucky State University employees are eligible for the drawing if they:
- Are a verified employee of Kentucky State University
- Have received at least their first dose of a Moderna, Pfizer or other World Health Organization approved 2-dose COVID-19 vaccine, or the one-dose Johnson & Johnson or other World Health Organization approved single-dose vaccine; and
- Have self-reported their vaccine information at the ADP Health Check Portal or by emailing a copy of their vaccination card (front and back) to human.resources@kysu.edu by the close of business on the day prior to the drawing. If you were vaccinated at a Kentucky State University affiliated site, you do not need to self-report unless asked to do so by Student Health Services.
How to upload vaccination status to ADP:
- If you are unable to upload the information into the ADP, please submit a copy of
your vaccination card (front & back) to the Office of Human Resources at human.resources@kysu.edu. All information received will be confidential.
- Open the ADP Mobile App on your device and log in.
- After logging in, users should see the prompt on the dashboard that says “COVID-19 Health Check-in” above Pay information. Select Respond.
- The next screen will display the question “Have you received a COVID-19 vaccination?”
with a note that says if a user responds they are fully vaccinated, they will be required
to upload photo proof of the vaccination on the next step. The options are as follows:
- Yes, I am FULLY vaccinated (it has been at least weeks since I received a single-dose vaccine or the second dose in a 2-dose series.”
- Yes, I am PARTIALLY vaccinated (I have not received the second dose in a 2-dose series, or it has been less than 2 weeks since I received a single-dose vaccine or the second dose in a 2-dose series.
- No, I have NOT received a vaccination.
- I prefer not to answer at this time.
- When an employee selects the option stating they are fully vaccinated, the next screen will display a calendar to select the date of their complete vaccination.
- After selecting the date, it will prompt the user to add a photo of the front of their vaccination status. Once the user adds the photo, it will provide an opportunity to re-submit the photo or submit it as-is.
- The user will be prompted to either submit a photo of the back of the card or select “my proof only has a front”. Users will again have the opportunity to re-submit the photo.
- The user will receive a confirmation message. The employee can select Done.
- Under the COVID-19 Vaccination Survey section, the user should see the status updated to “Complete” and the date the survey was completed.
To win, the entrant’s vaccination information and student or employee status must be verified by Student Health Services, Human Resources and the Bursar’s Office.
General rules
Winners of the drawings will be randomly selected from all eligible entries received and subject to verification of eligibility and compliance. Upon the random selection of an entrant and verification, the entrant will be declared the official winner of the drawing.
Eligible Kentucky State campus community members can only submit one entry for the drawings. Prior to each drawing, all duplicate entries will be removed.
Eligible entrants will be eligible for all random prize drawings. All winners will be removed from future drawings. No participant may win more than one prize.
All entries received by the deadline will be included in all future drawings for prizes.
Prizes
Campus community members who do not meet eligibility requirements will not be eligible to win.
Prizes for students include:
- Two scholarships (tuition only) for one academic year
- Two scholarships (tuition, room and board) for one academic year
- Five cash prizes of $500
- 10 passes for free lunch for a year from Sodexo
- 10 $250 vouchers to the Winner's Circle Spirit Shop
- Two free wireless headphones and smartwatches
Prizes for employees include:
- Five cash prizes of $500
- 10 free parking passes
- 10 passes for free lunch for a year from Sodexo
- 10 $250 vouchers to the Winner’s Circle Spirit Shop
- 10 free season ticket passes for all athletics events
- Two free wireless headphones and smartwatches

Prizes are non-transferable to any other person, including friends or relatives.
Alternate winners
If a potential winner cannot be verified, refuses to accept the prize or cannot be located, an alternate winner will be drawn and declared the official winner upon verification.
A potential winner who refuses to accept a prize will no longer be eligible for any future drawings in the Shot for Bred campaign.
The degree of effort to locate a potential winner or decision to declare a potential winner ineligible is at the sole discretion of Kentucky State University.
Alternates will be used in the order in which they were randomly selected as vacated spots become available.
Tax and financial aid implications
Official winners will be responsible for any taxes associated with the prizes. Federal, state and local taxes are the sole responsibility of the official winners. All winnings will be reported to the official winners and the IRS, as required by law. All federal, state and local laws, applicable taxes, and regulations apply.
Information for winners of prizes will be shared with the Office of Financial Aid, as the value of some prizes must be counted as financial assistance received.
