Financial Certification for International Students (regular and short-term, non-exchange)

Financial Certification for Visiting Exchange Students (exchange students only)

F-1 Extension
If your I-20 will soon expire, and you wish to continue studying at KSU, you need to complete this form within 30 days of the I-20 expiration date.

Transfer Out
If you are enrolled at KSU, or are on OPT, and wish to transfer to another college or university, you must complete this form.

Purpose of Form: To report the change of address of an alien in the United States

Form I-102, Application for Replacement or Initial Nonimmigrant Arrival-Departure Record
Purpose of Form: For a nonimmigrant to apply for a new or replacement Form I-94 or I-95 Nonimmigrant Arrival-Departure Document.

Form I-134, Affidavit of Support
Purpose of Form: To show that visa applicants have sponsorship and will not become public charges while in the United States. The sponsor must file a separate affidavit for each applicant.

Form I-539, Application to Extend/ Change Nonimmigrant Status
Purpose of Form :
For nonimmigrants to request extensions of stay or changes from one nonimmigrant category to another nonimmigrant category. Please see the form's instructions for your specific nonimmigrant visa category. It is used by F and M nonimmigrants seeking reinstatement of status. It is also used by K-3/4 nonimmigrants to request extensions of stay and by persons seeking V nonimmigrant status while in the United States or extension of stay as a V nonimmigrant.

I-795, Application for Employment Authorization
Purpose of Form: Individuals who are temporarily in the United States and eligible for employment authorization may file a Form I-765, Application for Employment Authorization, to request an Employment Authorization Document (EAD). Other individuals who are authorized to work in the United States without restrictions should also use this form to apply for a document evidencing such authorization.
