This application packet is intended only for students who participated in the College Work Study/Part-time Employment/Need Based Work Program during Fall 09 or Spring 10. Students must have received a payroll check, Direct Deposit of a payroll check, or had their payroll check applied to their bill. Submission of the incorrect packet will delay processing.
Please print the following application packet and submit the completed packet to the Financial Aid Office no later than September 30, 2010.

Agreement to Hire form
W-4 Tax form and instructions
Direct Deposit form
All forms on the checklist must be completed and signed.

Within three business days of receiving the application packet the Financial Aid Office will send via campus mail the Authorization to Work form to your supervisor. Please contact your supervisor in order to sign this form.
IMPORTANT: Please remember that you may not start work until the Authorization to Work form has been received and signed.
